What is collected?
Our website includes an online booking component where patients are able to book their own appointments. When booking for the first time, the user will be asked for the following information:
- Name
- Contact information including mobile number and email address
This is the only information requested via the website. By inputting these contact details, you are consenting to us contacting you via telephone or email if amendments need to be made to the appointment you just booked or any future appointments. All other patient information is taken at the clinic and you will be asked to sign a consent and GDPR form informing you of how we use this information.
It is a requirement by law that we do not share or distribute any personal information to third parties unless we have your explicit consent. Acer House Practice website does not use Cookies and therefore does not store any preference data based on your searches or input. You will not be contacted with promotion deals or surveys, we contact only on the basis to discuss the appointments you have booked with us. If we need to contact you or someone else on your behalf later, that will only be done with your approval (such as referrals).
Regarding patient information and clinical notes, all personal information is kept on a password protected online system. A patient may request to see or have a copy of the information that we hold under the Data Protection Act 1998.
